Waiver of Liability Relating to COVID-19
By submitting this waiver, I, the submitter, do hereby acknowledge and understand the contagious and potentially deadly nature of the novel coronavirus, also known as COVID-19.
I voluntarily assume the risk that my family and I may become exposed to or infected by COVID-19 while on a SUMMIT OF LIGHT sponsored mini sabbatical, and that such exposure or infection may result in personal injury, illness, permanent disability, or death. I understand that the risk of becoming exposed to or infected by COVID-19 while on a SUMMIT OF LIGHT sponsored mini sabbatical may result from the unintentional actions, omissions, or negligence of myself and others – including, but not limited to, conference center/host location employees, volunteers, and/or other guests and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury, illness, damage, loss, claim, liability, or expense, of any kind, including, but not limited to, personal injury, disability, and death, that I or my spouse, or other family member may experience or incur in connection with our acceptance of a SUMMIT OF LIGHT sponsored mini sabbatical.
On my behalf, and on behalf of my spouse or other family member, I hereby release, covenant not to sue, discharge, and hold harmless SUMMIT OF LIGHT, its Board members, assignees, agents and representatives, of and from all claims, including all liabilities, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this Release includes any claims based on the actions, inadvertent omissions, and/or negligence of SUMMIT OF LIGHT, its Board members, assignees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after a SUMMIT OF LIGHT sponsored mini sabbatical.
I understand and agree to all of the above, and also agree that I and my spouse or other family member are able and willing to wear a face covering while indoors in common areas of the host location (other than while eating), and will adhere to all other COVID-19 prevention measures the host location has in place for my/our safety, as well as the safety of their staff and other guests.
